Digital clinical platforms for NHS primary care

DataMedic helps GP practices, PCNs and ICBs turn routine EMIS Web and SystmOne data extracts into disease-specific dashboards, actionable patient cohorts and measurable quality improvement cycles.

Built by clinicians and data specialists with deep understanding of NHS primary care workflows, SNOMED, QOF business rules, clinical coding and population health analytics.

Why DataMedic exists

Primary care data is powerful, but difficult to turn into action.

GP practices already hold rich coded clinical data, but turning it into safe, useful and actionable insight is not straightforward. Disease registers, SNOMED codes, QOF rules, pathology results, prescribing history and local pathway criteria all need to be interpreted correctly.

Clinical rules are complex

QOF indicators, disease registers, SNOMED clusters, exclusions, prescribing criteria and pathway rules can be difficult to translate into reliable cohorts without both clinical and technical expertise.

Manual analysis does not scale

Practices and PCNs often rely on searches, spreadsheets and manual validation to identify patients who are overdue, uncoded, uncontrolled or suitable for structured review.

DataMedic turns complexity into action

We convert structured primary care extracts into dashboards, actionable cohorts and repeatable review cycles that teams can use within their existing local processes.

How it works

From extracted data to measurable improvement

DataMedic is designed around practical NHS primary care workflows. It does not replace EMIS Web, SystmOne, AccuRx or local recall processes. It adds the intelligence layer above them.

Step 1

Identify

Analyse structured EMIS Web or SystmOne extracts using SNOMED-led clinical logic.

Step 2

Prioritise

Group patients into clinically meaningful cohorts, such as missed diagnosis, overdue monitoring, uncontrolled risk or pathway opportunity.

Step 3

Act

Create patient review lists for coding checks, pharmacist review, recall, outreach or local pathway activity.

Step 4

Measure

Refresh the data over time to show what has changed, where gaps remain and which cohorts need further action.

DataMedic supports clinical review and operational prioritisation. It does not replace clinician judgement or automatically update patient records.

What we build

One platform approach, multiple clinical priorities

Each DataMedic clinical platform follows the same principle: combine primary care data, clinical rules and operational workflow into a practical dashboard for local teams.

Clinical platforms

Disease-specific dashboards for areas such as obesity, CVD prevention, diabetes, respiratory care and other long-term conditions.

  • Overview and benchmark metrics
  • Practice-level dashboards
  • Actionable insight cohorts
  • Full patient population tables

Population health dashboards

Tools for practices, PCNs and ICBs to identify care gaps, variation, inequalities and groups requiring targeted review.

  • Missed or uncoded diagnoses
  • Overdue monitoring
  • High-risk patient groups
  • IMD and inequality segmentation

Why DataMedic

Built around the realities of NHS primary care

DataMedic is designed by people who understand both the clinical and technical sides of primary care data.

NHS primary care expertise

Built around real practice workflows, disease registers, clinical coding, QOF and pathway delivery.

EMIS Web and SystmOne knowledge

Designed around structured extract workflows from the clinical systems used by most GP practices.

SNOMED and coding expertise

Clinical definitions are built around coded data, SNOMED concepts, PCD refsets and disease-specific logic.

Patient-level action

Dashboards do not stop at charts. They help teams move from insight to patient review, recall and local action.

PCN and ICB scalability

The same platform approach can support individual practices, PCN-wide delivery and ICB-level population health priorities.

Governance-aware design

Built with information governance, data minimisation, role-based access and NHS expectations in mind.

Who we support

Designed for practices, PCNs, ICBs and responsible partners

For GP practices

Identify patients missing from registers, overdue for monitoring, uncontrolled against targets or suitable for structured review. Reduce manual search work and create clearer patient review lists.

For PCNs

Coordinate improvement activity across member practices, compare variation, support DES/QOF priorities and run targeted pathway work at network level.

For ICBs

Understand population health need, unwarranted variation, inequalities and pathway gaps across local primary care, with outputs that can support commissioning and improvement planning.

For life sciences partners

Support NHS-facing pathway improvement through DataMedic, helping fund digital tools and services that identify unmet need and support better patient care under appropriate governance.
